Restaurant Report Card: 22 restaurants fail health inspections in April 2019

PHOENIX — On the first Wednesday of every month, ABC15 takes action to share restaurant report cards from around the Valley. In April, 22 restaurants and food establishments failed to pass their health inspections with a "C" or better.

PHOENIX

Quick Stop Mart
3860 E. McDowell Rd., Phoenix AZ 85008
Grade: D; see inspection report here.

Violations: During inspection of kitchen, employees running kitchen were unaware of how to properly date mark, proper hand-washing, proper temperatures for cold and hot holding, and lacked any knowledge of BIG5 signs and symptoms of foodborne illness; establishment was using a portion of cut tree trunk as cutting board; no soap available at hand-washing sink.

Tomaso’s
3225 E Camelback Rd., Phoenix AZ 85018
Grade: D; see inspection report here.

Violations: A pan of cooked potatoes was uncovered in the walk-in fridge and had a significant amount of mold growing on the surface; observed several single service grocery store "thank you" plastic bags used to store raw fish and shellfish shells; observed raw sausage stored above raw veal and portioned pasta; several bottles and bubble packs of pills were stored in the kitchen on shelves directly above food contact surfaces.

Giuseppe’s on 28th

Violations: Observed an employee touching carrots and other salad ingredients with their bare hands while making salads; several pounds of raw chicken were stored in a used grocery store "thank you" bag, with the raw chicken directly contacting the bag; observed several employee personal items, such as cell phones and lighters, stored on shelves directly above food contact surfaces in the kitchen.

Mellow Mushroom

Violations: Observed ready-to-eat fruit (cut pineapple, containers of strawberries), and ready-to-eat chocolate chip cookie placed in same container as raw bacon in the prep refrigerator; breakfast menu had an incomplete consumer advisory.

Hilton Garden Inn Phoenix

Violations: Upon arrival, inspector was informed by employees that there was no person in charge; in the kitchen, observed an employee begin washing hands while a band aid was still attached to their finger; in the dish area, observed a dishwasher handling dirty dishes with their bare hands; observed a pan of raw bacon stored directly over yogurt parfait cups and an open box of lemons.

Hana Sushi Restaurant

Violations: Observed employee rinse hands for 2 seconds after handling raw chicken; large white jug, sriracha bottle, and spray bottle all found containing cleaning chemicals without any labeling.

Salon Acapulco

Violations: Observed raw shelled eggs stored above containers of clam/tomato juice, raw tuna stored directly on top of an unsealed package of cut lettuce, and raw bacon above ready-to-eat salsas; several porous stone bowls were stored as clean with dried food debris stuck to the inside and outside surface of the bowl; several roaches were found crawling on a wall in the kitchen and inside of the gaskets of the kitchen prep line fridge.

Carniceria El Rancho Grande LLC

Violations: Observed a large handheld immersion blender stored in the clean dish area, had a build up of organic matter on the blades; near a prep sink on a shelf, observed two cans of Raid insect repellent; unlabeled desserts in grab-and-go area.

Filiberto’s Mexican Food

Violations: The establishment does not have a person in charge present; observed raw shelled eggs stored directly on top of prepped to go containers of salsa; observed a spray can of raid pesticides stored underneath the three compartment sink.

La Carreta De Lily Elotes Y Neveria

Violations: There was various Ziploc bags full of cooked shrimp inside of the walk-in refrigerator. When requested, the operator was unable to present an invoice, a receipt, or original packaging for the shrimp; the handle of a large scoop stored within ice for consumption inside of black/red cooler that is between the two ice cream display cases was making direct contact with the ice.

Filiberto’s Mexican Food
2001 S. 7th Avenue, Phoenix AZ 85007

Violations: Observed a large 5-gallon bucket containing red salsa. It was measured at 60 degrees F, and had been made the previous day; in the reach-in across from the grill, observed a container of liquid eggs, measured at 67 degrees F.

EAST VALLEY

Grandview Steakhouse Restaurant & Lounge (Painted Mountain Golf Resort)
6210 E. Mckellips Road, AZ Mesa 85215

Violations: Observed dirty dishes in handwashing sink next to coffee/tea station; observed dishwashing machine in kitchen and bar area dispensing 0 ppm of chlorine sanitizer; observed pot roast and meatballs in walk-in cooler from 4/1/19 (2 days past their discard date).

Senor Taco

Grade: D; see inspection report here.

Violations: Observed food handling employee at the cook line wiping their gloved hands on their dirty apron in between handling ready-to-eat foods; observed a metal strainer and two knives stored as clean, but still had food debris dried on it; some containers still had old date marking stickers on them; damaged or disrepaired equipment store on a shelf (ie: twisted cable-style cheese block slice, white rubber spatula, melted plastic containers).

Valero Grab N Go

Violations: Observed 3 compartment sink being used to store caution tape and plastic bucket; observed no soap at handwashing sink in stockroom; observed squeegee being stored in handwashing sink in stockroom; handwashing sink was observed to have not been used in quite some time due to heavy buildup of hard water scale and dust.

Salvadoreno Restaurant
303 E. Southern Ave., 113 Mesa AZ 85210

Violations: No certified food manager card was available at the time of inspection and PIC was unable to demonstrate knowledge relating to prevention of foodborne illness at the time of inspection; observed no current food handler certificates for any employees at the time of inspection; observed employee crack raw shell eggs and put on gloves to begin handling RTE (Ready To Eat) foods without washing hands; observed employee dump cooked meat into kitchen hand wash sink.

Salt & Lime Modern Mexican Grill

Violations: On cook line, employee texting on phone, then started working with cooking utensils with same gloved hands; at bar, employee handle cut fruit for customer drink with bare hands; in walk in cooler, container of shelled eggs stored above ready to eat fruits and vegetables.

Violations: Observed mouse droppings on top of mechanical dishwasher, and on floor underneath dishwasher and in dry storage room behind dishwashing area; observed beverage container being stored in ice bin at bar; observed employee drinks being stored on prep table shelves; observed employee drink from cup then handle to-go containers and Ziploc bags at the time of inspection.

Jin Shabu

Violations: Observed employee tap employee on the back and touch face then proceed to handle clean equipment for portioning out syrup without washing hands; observed raw, shaved beef stored above cooked shellfish and raw fish; observed no hot water available in the establishment.

Urbanh

Violations: Observed employee food throughout the walk-in cooler above and comingled with customer food; observed magnetic strip with buildup of organic debris on top and face where knives are stored; observed homemade vegetable slicer fashioned from slicer glued to the inside of a container lid.

Ruchi Indian Cuisine

Violations: Live German roaches in glue traps; only one valid food handler certification at time of inspection; employee handled dirty dishes then clean dishes without washing hands; two employees went outside and came back inside and started to handle food without washing hands; employee put on gloves to handle food without washing hands; accumulation of food debris on the can opener blade and holster, condiment containers.

Backyard Taco

Violations: Observed employee handle raw meat at the grill with gloved hands and then proceeded to touch cooked meats without changing gloves and washing hands; observed raw meat on grill touching cooked meat.

Beast of Bourbon Bar and Grill

Violations: Worker reporting to work began scooping ice at the servers station soda fountain ice bin and dispensing a personal drink prior to washing hands; no hand soap available at the bar hand washing sink.

WEST VALLEY

All restaurants in the West Valley inspected in April passed their health inspection with a "C" or better.
