PHOENIX — A construction worker who was killed in a “workplace accident” at the Arizona State University downtown Phoenix residence hall construction site on March 17 was crushed when a crane dropped a load of concrete forms on a deck of the building, causing it to cave in, according to a police report.

According to the report, obtained by the Phoenix Business Journal through a public records request, Braxton Ulmer, 27, an employee of Suntec Concrete, was struck on the west side of the building site, located near Second Avenue and Fillmore Street.

According to the report, Ulmer was working on a second story deck that was hit by the falling concrete forms. The deck collapsed and Ulmer was covered with debris from the load and the fallen deck.

Ulmer was transported to Banner University Medical Center where he was pronounced dead.
