The FBI launched an investigation into a massive, far-reaching data breach involving a Florida-based healthcare provider that has physicians in the Valley.

21st Century Oncology notified patients that their information may have been stolen by an unauthorized third party. More than 2.2 million people may be affected.

According to the company, a forensics firm determined that the intruder may have accessed the database on October 3, 2015.  Patients were not notified sooner, the company said, because the FBI was concerned it could interfere with their investigation. Information accessed may have included patients’ names, social security numbers, physicians’ names, diagnosis and treatment information, and insurance information. The company said there is no evidence that any medical records were accessed.

According to the 21st Century Oncology physician finder, there are 15 doctors in the Phoenix area affiliated with the company.  You can see if your doctor is one of them by clicking on the physician finder.

21st Century Oncology said there is nothing to suggest the information has been misused, but it is offering free credit protection services to affected patients.
