Restaurant Report Card: 17 Phoenix-area businesses failed their health inspections in November

PHOENIX — On the first Tuesday of every month, ABC15 takes action to share restaurant report cards from around the Valley. In November, 17 restaurants or food establishments failed to receive a "C" or better on their health inspections.

See inspection details from the Maricopa County Public Health Department below (violations taken verbatim from reports). Most violations were corrected at the time the violation was observed, according to the reports.

PHOENIX

Dijlah Market
8048 N. 27th Avenue, Phoenix AZ 85051
Grade: D; see inspection report, here.

Violations: Observed bastorma (Arabic type sausage), in glass freezer case near front of meat department without any identifying origin labels. Person in charge stated they purchased the sausage from a distributor in San Diego and another distributor in Phoenix, however they were not able to locate a receipt or invoice demonstrating the sausage was produced from an approved source; Observed an aerosol can of "Raid" insecticide spray being store inside mop sink in meat processing area. Advised person in charge (PIC) that pesticides should not be stored on the premises. PIC removed pesticide can; Observed boxes of latex gloves under prep table in meat department. Advised person in charge that only vinyl, food grade type gloves are to be used when handling food.

Carniceria El Ranch Grande LLC
4227 S. Central Avenue, Phoenix AZ 85040

Violations: At the doorway near the grill and the prep sink, observed over 5 live cockroaches crawling from within the door frame. PIC will have pest control come out and spray this area; Above a flat top grill, there is an electric fly catcher directly above. Please move the fly catcher to an area where it is not above food preparation; Throughout the deli and bakery, observed multiple eclectic fly catchers that were completely full of dead flying insects. Please replace the sticky pad to allow the device to catch more flies; In the storage area near the grills and main cooking area, observed a build up of food debris and organic matter underneath all of the shelves. Please deep clean this entire area to remove the debris and build up in order to not attract pests; Observed a room that is attached to the deli are, near the prep sink and the grill. Upon entering this room, observed old and unused equipment, chemicals, and litter scattered throughout the room. Please remove all unused item from this area in order to prevent the harborage of pests.

Schlotzsky’s
4445 E. Thomas Road, Phoenix AZ 85018

Violations: The establishment does not have a certified food manager; An employee touched an customer's credit card with gloved hands. The employee removed their gloves, washed their hands for approximately 6 seconds before returning to the kitchen and putting on gloves; The handwashing sink by the baking area and the three compartment sink was obstructed by a speed rack. All handwashing sinks MUST be maintained free and accessible at all times to allow for proper handwashing. The PIC moved the speed rack; A slicer was stored as clean with dried food debris stuck between the pushing tines; All TCS items in the top of the make-line prep fridge were out of temperature. Cooked and deli sliced beef, ham, turkey, and chicken, as well as provolone, mozzarella, swiss, cheddar, cooked mushrooms, and cooked red bell peppers were between 48*F-55*F.

La Consentida Tacos y Sushi

Violations: Observed food service employee wearing gloves, mopping floor and washing dishes then returning to kitchen to handle food without washing hands and changing to new gloves; Hand washing stations in kitchen and back dishwashing/store room area did not have a means to wash dry hands after handwashing; Handwashing sink across from prep sink and in back dishwashing/store room area did not have hand soap; Observed container of partially cooked chicken wings being stored above container ready to eat foods in reach refrigerator in back kitchen area near stove/grill. Advised person in charge (PIC) that that the partially cooked chicken wings are still considered a raw meat and must be stored below or away from ready to eat foods such as tomatoes; Front of restaurant prep-refrigerator/sushi making area has plexiglass type barrier in front and on sides of of prep area. However, an additional barrier needs to be installed on side and in between the plexiglass prep area barrier to prevent contamination.

Pho Lee Restaurant

Violations: During inspection there were multiple cooling/cold holding violations and person in charge (PIC) failed to notice or acknowledge that their employee had both caused and also failed to correct violations more than once with the same food items during the inspection; Handsink in kitchen area was blocked with prep carts and equipment at time of inspection, handsinks was cleared at time; Observed large covered containers of cooked pho broth cooling on floor below prep table, items measured between 100-120 degrees F. Employee said items had been cooked recently and were put there to cool, had items moved to cooler at time; Observed strainer filled with cooked noodles sitting on chest freezer in splash range of roof leak from recent rainstorm, had item moved to a cooler at time. Additionally, observed frozen meatballs and containers of cooked broth being stored on floor below sink and prep table in kitchen area.

Padre Murphys

Violations: Observed cooked sliced turkey (made 11/18) and tomato based sauce for Bruschetta (made 11/19) past discard date. Person in Charge discarded out of date Time/Temperature Control for Safety Foods; Observed prep top missing hardware along top hinges and container for chips in disrepair. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under Parts 4-1 and 4-2.

Kneaders Bakery and Cafe
4126 E. Indian School Road, Phoenix AZ 85018

Violations: At time of inspection the handwashing sink by the 3 compartment sink was used to store a rag and the handwashing sink by the baked goods was used to store a spatula; At time of inspection there was broccoli cheddar soup being hot held at 122*F. PIC stated that the soup had been hot held for around 2 hours and likely someone turned down the temperature too low after the soup reached 165*F; At time of inspection there were dinner rolls being stored out on the dining area on the customer tables. These rolls were susceptible to contamination by customers and were discarded at time of inspection. There was also self serve lemons without a cover on by the soda fountain, these were also discarded; At time of inspection there was fresh lettuce being stored in black garbage bags. An employee stated that they were using the bag to mix the salad. PIC was advised to use either a food grade bag or a large container to mix the lettuce.

EAST VALLEY

Pei Wei Fresh Kitchen

Violations: Observed sushi with raw tuna on a rack in the walk in refrigerator above a tray filled with RTE crab mix. PIC moved the sushi to the bottom of the rack. Raw animal foods must be placed below ready to eat foods; Observed food debris on plastic food containers on the clean dishes shelf. PIC moved the dishes to the 3 compartment sink to be re-washed. Equipment must be stored clean. *2nd repeat violation; Observed no paper towels at hand sink near the entrance to the cooking area. PIC refilled the paper towel dispenser. Hand sinks must be supplied at all times.

Thirsty Dog 2 Go
15 W. 5th Street, Tempe AZ 85281

Violations: Observed hot dogs and TCS sauces holding at 109* Fahrenheit. PIC stated the hot dogs had been placed in crock pots to be reheated approximately 2 hours earlier. PIC moved hot dogs to steamer to rapidly bring up to temperature; Observed cream cheese spread held at 55* Fahrenheit and cut tomatoes holding at 49* Fahrenheit. Also observed shell eggs holding at 54* Fahrenheit and guacamole holding at 55* Fahrenheit. PIC voluntarily discarded cream cheese and tomatoes as they had been above 41* Fahrenheit for longer than 4 hours. PIC moved eggs and guacamole to walk in refrigerator to rapidly cool down as they had not been held at 41* Fahrenheit for longer than 4 hours; Observed mechanical dishwasher utilizing chlorine sanitizer measuring 0ppm at time of inspection. PIC stated a work order would be placed to maintain dishwasher and placed a "Do Not Use" sign on the dishwasher.

Cobblestone Car Wash

Violations: No certified food manager on staff with many issues cited and "person in charge" at time could not correctly answer related specific food operation questions; Foods including prepackaged burritos, sandwiches, and cheese sticks found in open self-serve display case at 54F to 61F - foods embargoed at time. See embargo form. Maintain all cold held potentially hazardous foods at 41F and less; Cleaning log(s) for the F'Real milkshake mixing unit not available - manager provided a blank log sheet at time and stated the worker responsible for maintaining the log was not available at time; Open self-serve sandwich display case found operating at >50F - repair. Operator must not use this unit for the storage of any potentially hazardous foods until repaired and maintaining them at 41F and less; Large dog observed being brought into the store by a patron without a visible disability did not elicit a response from workers even when animal began behaving like a pet and not a trained service animal - trained manager on duty the requirements for service animals and the responsibility of staff to inquire as to the reason for the animals presence.

The Enclave at Gilbert Senior Living

Violations: Observed grill employee with gloves touch raw hamburger patty. With same gloved hand; grill employee began preparing tuna salad plate. Stopped employee. Reviewed events listed previously. Employee took off gloves, discarded tuna salad; washed hands for more than 20 seconds; Observed multiple TCS foods past 7 days of expiration in the establishment. Mozzarella cheese bag opened on 10/25; expired on 10/31; pan of Turkey deli slices with date mark of 11/4; expired on 11/10; container of tuna with date mark of 10/31; expired on 11/6. Notified Person in Charge (PIC); PIC confirmed and verified expired products. PIC discarded all expired products; Observed potato soup hot holding inside hot holding unit with internal temperature of 112*F-116*F; notified Person In Charge (PIC); PIC verified and confirmed temperatures. PIC reheated soup on stove top.

Subway

Violations: Observed employee wash hands for 5 seconds. Discussed proper handwashing procedures with employee at time of inspection; Observed employee cough in to hands and then proceeded to work with utensils. Discussed with employee about when to wash; Observed cooked steak in prep-table with an internal temp of 50F. employee stated that steak had been there all morning. Discussed proper cold holding temps with employee at time of inspection. Steak was discarded; Observed no light bulb in walk-in freezer at time of inspection.

Yuliana’s Mexican Food

Violations: Observed several personal drinks without both a lid and a straw. All personal drinks need to have both a lid and a straw and to be located in an area that will not contaminate any food, food prep, or utensils; Observed food being stored at the 3 comp sink. Discontinue this practice. All food prep needs need to be done at the food prep sink or food prep tables; Observed several wet rags left out in the kitchen. Discontinue this practice.

The Summit at Sunland Springs Memory Care

Violations: Observed employee wash hands for 1 second-5 seconds. Stopped employee. Reviewed steps to wash hands. First wet hands, apply soap, scrub for 10 seconds, dry hands, turn off handwashing station with paper towels; Observed employee touch mouth with left fingers. Employee put toast into toaster with bare hands. Employee grabbed 4 slices of toasted toast with bare hands and cut them in half while touching with bare hands. Employee plated toast slices onto plates with bare hands. Employee after plating toast again touched toast with bare hands. As employee began to take food to dining hall stopped employee; Observed deli slicer in back kitchen area with heavy organic pieces and particles on back of blade and green liquid residue on front of blade; Observed several food items in hot box not at proper hot holding temperature; Observed inside reach in refrigerator in-house prepared hummus with date mark of preparation on 11/8/19; todays date is 11/18/19. Observed in main walk in refrigerator two big cooked hams prepared on 11/11/19 with discard date of 11/17/19; todays date is 11/18/19.

Arby’s
953 N. Dobson Road, Mesa AZ 85201

Violations: Observed an open box of packaged mints and cardboard sandwich containers stored on the shelf below floor cleaner, detergent, and next to chlorine. The open box of mints and sandwich containers were moved to a different shelf, away from the chemicals; Observed a container of cooked, breaded onions stored on the make-line with a label indicating a 10 pm discard time (10 hours later). According to PIC, time as a control is used for the cooked onions and it should be labeled for 4 hours. PIC discarded the onions, see embargo form; Observed cut cabbage and lettuce, sliced tomatoes, and various sliced deli meats in the cold well with an internal temperature of 47-50F, probe thermometer, verified by person in charge (PIC). Also observed hashbrowns, prepped salads, whipped cream, and containers of milk with internal temperatures of 48-50F, cold holding in the front counter reach-in.

Basil & Garlic Bistro
9330 E. Poinsettia Drive, Scottsdale AZ 85260

Violations: On cook line, employee handled ready to eat parsley for ready to eat garlic bread without gloves. Instructed employee to discard garlic bread, wash hands, and don gloves when working with ready to eat foods; On flat top stove, marinara with meatballs with an internal temperature of 111*F. Per employee, heat was turned off stove <4 hours. Employee turned flat top stove on to re-heat marinara with meatballs to 165*F and maintain at proper hot holding temperature; In walk in cooler, lasagna, open package of meatballs and in bottom portion of large prep table cooler, containers of cooked pasta without proper date marks. Per employee, lasagna was made made 4 days ago, meatballs were opened 3 days ago, and pasta was cooked 3 days ago. Instructed employee to properly date mark lasagna, meatballs, and cooked pasta;

Nishikawa Ramen

Violations: Paper towels not available for front employee hand sink; Observed a container of raw chicken stored over a box of raw cabbage in the walk in unit; Vegetable egg rolls and shrimp dumplings cold holding at 45 deg F PIC discarded. Bean sprouts, cooked corn and cooked pork cold holding at 47-49 deg F in reach in unit holding out of specification. PIC informed the food items had been placed in this unit a short time prior from walk in; Observed reach in unit on the right side holding at 48.6 deg F for left side of unit and 52.3 deg F for right side. TCS food items was redirected to alternate cold units Will re-inspect.

WEST VALLEY

No restaurants in the Valley inspected during the month of November scored a "D" on their health inspections.
