Restaurant Report Card: 15 Valley restaurants failed health inspections in December 2019

PHOENIX — On the first Tuesday of every month, ABC15 takes action to share restaurant report cards from around the Valley. In December, 15 restaurants or food establishments failed to receive a "C" or better on their health inspections.

See inspection details from the Maricopa County Public Health Department below (violations taken verbatim from reports). Most violations were corrected at the time the violation was observed, according to the reports.

PHOENIX

Bahia de Guaymas Restaurante
4220 S. 16th Street, Phoenix AZ 85040
Grade: D; see inspection report, here.

Violations: Observed uncovered raw fish ceviche above soft drinks and salsa in the walk in. Had the PIC move the ceviche below the items to prevent contamination; -Observed an open bucket of a pink liquid below the handwashing sink next to the freezer without a label. Per PIC it is a degreaser. Had the PIC label and cover the degreaser at time of inspection; Observed open bucket of fish sitting on the floor of the walk-in. Had the PIC move the fish on to the shelf so that it could be properly stored; Observed a pipe leaking below the sanitize portion of the 3 compartment sink. Discussed keeping plumbing in good repair with PIC.

Restaurante Cancun

Violations: Several packages of tortillas had holes in them, and the tortillas appeared to have been partially eaten by small rodents. Had employee discard these packages; Observed an employee wash hands in a 3-comp sink. Explained to all employees that hands must be washed in a hand sink and ONLY a hand sink; Several cooking/preparation items were being stored on the floor and in the mop sink. Food contact surfaces should not be stored in an area in which contamination can result; Several pounds of cooked meats were stored in what appeared to be trash bags. No employee was able to determine what kind of bag the food was stored in, and no containers of the bags were available to determine whether they were food-grade or not.

The Clarendon Hotel & Spa

Violations: Observed employee grab visibly soiled sprayer in warewashing area, rinse off a spatula, dry spatula on visibly soiled apron, and grab clean utensil from rack with the spatula still in hand. When brought to attention, employee placed both utensils in warewashing area to be properly cleaned and washed hands for 20 seconds; Observed sofritas and cooked pasta in right prep cooler with date stickers from 11/29 and 12/2, respectively. Date of inspection is 12/10. Per PIC, both items were prepared 2 days ago. When brought to attention, PIC voluntarily discarded items; Observed in-use utensils on grill in 122 degree F water. When brought to attention, employee discarded water and placed utensils in water that had just been boiled. Ensure in-use utensils in water stay above 134 degrees F or below 42 degrees F.

Violations: An employee was observed discarding a beverage into the handwashing sink near the dishwashing area. PIC was advised that handwashing sinks are to be used ONLY for handwashing; Automatic dishwasher that uses chlorine had a concentration of 0 ppm at time of inspection. The machine was currently in use and had dishes inside. PIC was advised to have dishwashers check the concentration of chlorine in the dishwasher at least two times per day and to ensure that it has a concentration of 50-100 ppm; In the cold holding unit across from the stove there was a container of fresh garlic and oil dated 11/29. PIC was advised that a garlic in oil mixture is required to be discarded within 7 days including the day it is prepped. PIC discarded the mixture at time of inspection.

Taco Feliz Mexican Food

Violations: Two boxes of moldy strawberries and a package of moldy cheese were found in a box with limes. Per PIC, the strawberries are not used at the establishment, but the cheese is; No date marks were observed on any food items in the walk-in. Per PIC, containers of cooked chicken, shredded beef and rellenos were made three days prior to inspection. PIC placed date marks on these items at time of inspection; Several bottles and buckets of unknown chemicals were not labeled. Had PIC label them at time of inspection. Please ensure that chemicals being stored in a container other than the original one are labeled.

EAST VALLEY

Virtu

Violations: Observed raw duck leg stored above raw pork in the outside walk-in refrigerator, discussed food storage chart with the manager, manager moved the raw duck leg to the bottom shelf; Observed cooked sliced ham date marking as 11/22 in the outside walk-in refrigerator, Discussed date marking procedures with the manager; Bottle of Excedrin stored on counter with food containers and utensils. Manager removed medicine to proper storage.

Isabel's Amor

Violations: Observed employee with gloves on handling personal cell phone and then working with open foods. Had employee remove gloves and wash hands. Discussed "when to wash" with person in charge and employee; Observed window cleaner and degreaser in spray bottles stored hanging on rack in bar next to and above clean utensils. Discontinue this practice; Establishment does not have chlorine testing strips for both dish machines. Get testing strips prior to re-inspection.

Wendy's

Violations: Observed an employee wash their hands for approximately 3 seconds after touching raw ground beef. The employee then dried their hands and attempted to return to the cook line; Observed an employee touch raw chicken with a single glove on their hand. The employee then removed the glove and touched a spatula for raw beef and a wiping cloth in sanitizer solution with the contaminated hand. The employee used the wiping cloth to clean a nearby counter; The handwashing sink near the three compartment sink did not have any available paper towels, or any nearby appropriate hand drying method.

Joe’s New York Pizza

Violations: Observed a large whisk being stored in the hand wash sink. Establishment only has one hand wash sink located in the kitchen. Informed owner and employees that hand wash sinks shall only be used for the purposes of handwashing; Observed no soap at the only hand wash sink in the kitchen; Observed no paper towels at the only hand wash sink in the kitchen; Observed a container of raw chicken being stored in the reach-in on the shelf above cooked chicken wings. Observed a carton of dozen raw shell eggs in walk-in on top shelf above cooked pasta, ready-to-eat sauces, cheeses, and ready-to-eat deli meats. Informed owner and employee that raw chicken and eggs shall be stored below cooked and ready-to-eat foods; Observed vegetable peelers and large tomato slicer being stored as clean on shelving unit with dried accumulated organic debris resembling tomato skin and seeds.

Three Wiseman

Violations: Observed the handwash station behind the bar without hand soap; Observed back-of-house dishwasher with no chlorine; Observed containers of garlic parmesan dressing/dip and cooked black beans in walk-in with date marks reading 11/26 and 11/24, respectively. All cold-held TCS foods shall be kept for no longer that 7 days; after this time the items shall be discarded; Observed chopped lettuce (in walk-in) and sliced ham (in reach in cooler on cook line) without date marks; Observed an unlabeled spray bottle filled with blue liquid underneath the 3 compartment sink in the dish put. According to person in charge, the bottle was filled with glass cleaner.

Dragon Express

Violations: Observed employee handling raw chicken with over glove, remove it with other hand then handle fryer equipment without removing gloves and washing hands. Employee washed hands at the time of inspection (repeat violation); Observed employee cutting lettuce bare handed. Lettuce was being cut for garnish that comes in direct contact with crab puffs. Employee discarded lettuce, and donned gloves at the time of inspection; Observed knife with caked on food debris being stored with clean knives. Knife was not in use at the time of inspection; Observed fork and wok scoop handles with cracked, split and splintering wooden handles. Utensils were discarded at the time of inspection.

China Way

Violations: Observed employee food being stored on top shelf in walk-in cooler above customer food. Person in charge moved at the time of inspection; Observed employee dispense bleach for sanitizing then handle equipment for removing chicken from deep fryer without washing hands; Observed employee use wok spoon to spoon pooled eggs from container then use same spoon to spoon stir fry vegetables from prep table. Person in charge cooked vegetables to 165 *F at the time of inspection and manager provided a second spoon for employee to use for ready-to-eat products.

Casa Amigo’s and Skylanes

Violations: Observed employee in dish-pit handling clean dishes/equipment after handling dirty dishes. Employees are to wash hands after handling soiled equipment and before handling clean equipment; Observed employee at bar washing hands in a dump sink that was next to a designated handwash station; Observed the handwash sink at the north bar downstairs filled with various pieces of garbage. Observed hand sink in dish-pit with food debris and garbage in the basin.

Rigatonys

Violations: Observed handwashing sink at cookline blocked by trashcan and magnetic strip/knives, and being used to store dry cloths. Person in charge removed cloths and trashcan to make sink accessible to staff; Observed shellstock tags from December and October with no last sold dates. Person in charge will ensure shellstock tags are maintained with the lot until the date the last shellstock is sold from the case, remove tag, mark the date and maintain in chronological order for a minimum of 90 days; Observed container of manicotti stored below containers of raw meat pasta and raw seafood. Person in charge moved manicotti at the time of inspection.

WEST VALLEY

Yellowtail Sushi Steak

Violations: The manager touched the trash and the and handled clean utensils with out first washing his hands. The manager was instructed to wash his hands which he washed his hands and donned gloves; Handwashing. The handwashing sink by the mechanical dish machine was found contaminated with food and blocked by a freezer. The freezer was moved to allow access to the sink; Dry cloths contaminated with food are being used to wipe down food contact surfaces; There is a large accumulation of grease and grime on the exterior surfaces of the oven, stove, and fryers, The shelving surfaces above 3-compartment sink and above grill have a buildup of food. The wall and floor junctures throughout the kitchen are coated with a mixture of old food, dirt, and grease. Containers and trays holding clean utensils are dirty and require cleaning. Containers holding spices are coated with oil and very dirty.
