ABC15 is taking action to keep you and your family updated about local health and health care issues.

Leapfrog Hospital Safety Score recently released its rankings of all Arizona hospitals, from those scoring A grades to others falling behind.

Four hospitals on the list received a D grade: Banner University Medical Center in Tucson, Canyon Vista Medical Center in Sierra Vista, Kingman Regional Medical Center* in Kingman and St. Luke's Medical Center in Phoenix.

Twenty-six hospitals were given a C grade. To see if your hospital is on the list, click here.

*Editor's note: Kingman Regional Medical Center has issued a statement saying data was incorrectly reported to the U.S. Centers for Medicare and Medicaid Services, which is one place Leapfrog gets its data. "We appealed to CMS to correct the data and were refused. With the help of the American Hospital Association, KRMC is currently appealing for a second time," the hospital said.
