Woman missing from Peoria after smashing phone

Police are asking the public to help locate a missing woman. 

Peoria police say Vivian Jacks, 58, went missing on June 8 around 5:15 a.m. Officials say the woman left her home near 75th and Peoria avenues after smashing her phone. The woman did not tell her family where she was going and has not returned home since then. 

Officials say the woman has been depressed and may be suicidal; she's been on medication and has missed several appointments. She has also not shown up to work. 

The woman is believed to be driving a silver 2001 Ford F-150 pick up truck with a super crew cab. The license plate on the vehicle is "436LFL." A description of the woman's clothing is unavailable.
