One person is dead following a crash in west Phoenix on Wednesday.

The incident happened on 51st Avenue near Interstate 10 around 1:30 p.m.

Phoenix police said a van with six people inside, including three children were struck by a vehicle exiting the freeway.

The force of the crash caused the van to roll onto the driver's side. The front passenger, 69-year-old Jesse Morales, died from his injuries. The driver suffered serious injuries. Another adult and the three children inside were uninjured.

The driver that crashed into the van stayed on the scene and did not show any signs of impairment.

Crews transported a 39-year-old woman to the hospital in critical condition, and a 69-year-old man was also taken to the hospital in extremely critical condition.

The children in the van range from 2 to 11-years-old and were taken to the hospital in stable condition by family members, fire officials said.
