Gilbert police are asking for the public's help in locating a missing 60-year-old woman.
Police say Evelyn "Cindy" Guido was reported as missing on April 12.
Guido reportedly left her home in Gilbert on April 6 around 8:30 p.m. and has not had any contact with her family since.
She was supposed to have a medical procedure on April 13 but never showed up for the appointment, police said.
Police say Guido has serious health concerns and physical disabilities that require ongoing treatment.
Authorities say they have not been able to find evidence of social media posts, financial transactions, or contact with family since her disappearance.
