Breast Implant Options by Steven H. Wiener, M.D.Board Certified Plastic Surgeon

Breast augmentation with implants is one of the most popular cosmetic surgical procedures in the United States. Each year approximately one half million women undergo breast augmentation in the United States. There currently are three types of breast implants available in the United States. All of these implants are approved by the Federal Drug Administration (FDA). Saline implants are filled with sterile saline (salt water). Silicone implants are filled with silicone gel. These implants have a more natural feel. Gummy bear implants, also known as form stable implants, because they maintain their shape. The silicone gel in gummy bear implants is thicker and more cohesive, and tends to be firmer than traditional silicone implants. All implants come in round and shaped styles, and projections. Saline and silicone implants both have advantages. Both types of implants produce excellent long term breast augmentation results. Saline implants are constructed of an outer silicone shell, and include a fill port. They can be placed through a very small incision, and then inflated when in good position. Saline implants are less expensive than silicone implants. They tend to feel firmer and less natural than silicone implants. Saline implant volume is adjustable, and allows for volume correction in breast size discrepancy. Saline implants are more likely to have palpable wrinkling or rippling that can be felt along the edge of the implant. Silicone gel implants have a strong outer shell of solid silicone, with a softer, gel-like central core. Silicone implants used today have a semi-solid (jello-like) material that rarely leaks. Silicone gel implants are more expensive than saline implants, and feel more natural and softer than saline implants. Silicone gel implants are much less likely to develop edge wrinkling than saline implants. Silicone gel implants come pre-filled, and require a longer incision to implant. Gummy bear implants or form stable implants come in pre-determined shapes and profiles. These currently are the most expensive implants available. They maintain their shape or profile; however, they are the firmest type of implants. Gummy bear implants do not have edge rippling, but must be inserted into a very tight pocket. All implants come in multiple sizes, shapes, and profiles. All breast implants sold in the United States come with a limited manufacturer warranty.
