PHOENIX - Banner Health is investigating how sensitive member information got printed on its magazine labels.
The company is calling the mistake human error.
It's a mistake that scared about 50,000 Valley residents who found out about the error when they went to check their mail.
Imagine their surprise when they saw the address label on the Banner Health magazine had their Medicare ID or Social Security numbers right next to their address.
Banner Health tells ABC15, the magazine is sent out 2-3 times a year to people who are part of the Medicare Pioneer Accountable Care Organization.
The company says an internal file did not go through a standard review process causing the sensitive information to inadvertently be printed on the address labels.
Banner officials said they are changing their policies by making sure files aren't released without review and approval.
They are also offering credit monitoring services to anyone impacted.
Officials said details will be mailed out to anyone who received the magazine with their personal information on it.
