AmTrust Bank Caring Kids Contest

OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN. No deposit, purchase of a service or product, payment or bank transaction is necessary to enter or win and such activity will not increase your odds of winning. . Open only to legal residents of Arizona, who are between the ages of 5 and 17 as of date of entry.  Sweepstakes is subject to all applicable laws and regulations and is void wherever prohibited by law.  Employees of AmTrust Bank (collectively, “Sponsor”), their respective affiliates, subsidiaries, advertising and promotion agencies, suppliers and the immediate family members (spouse, mother, father, in-laws, grandmother, grandfather, brother, sister, children and grandchildren) and/or those living in the same household of each are not eligible to participate. By participating, entrants agree to be bound by the Official Rules and the decisions of Sponsor. 

TO ENTER:

1)      Beginning 12:01 PM Mountain Time) February 4, 2013 through 11:59 AM (Pacific Time) July 31, 2013 (“Contest Period”) go to www.abc15.com/caringkid and follow the on-screen instructions to enter.

OR

2)      Mail in a copy of the story detailing the Caring Kid and their story complete with name, street address (no P.O. Boxes), city, state, zip code, entrants date of birth (mm/dd/yyyy), plus daytime and evening telephone numbers. (If you do not have a telephone, state “No Phone.” Failure to comply with this or any other official rule will result in disqualification). Mail your entry in a hand-addressed, business-size envelope (#10) with first-class postage, to: AmTrust Bank, 1801 E. 9 ^th Street, Cleveland, OH 44114. Entries must be received by the 15 ^th of each month. Limit one entry per envelope.

Limit one entry per person/household address regardless of method of entry .  No mechanically reproduced entries of any kind are permitted. Illegible and incomplete entries are void. Sponsor is not responsible for lost, late, mutilated, postage due, misdirected entries or entries not received. In the event of a dispute over who submitted an online entry, the entry will be deemed to have been submitted by the authorized account holder of the e-mail account identified on the online entry form. The authorized account holder is deemed to be the natural person who is assigned to an e-mail address by an Internet access provider, service provider, or online organization responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.  Entries become the property of Sponsor and will not be returned.

DRAWING:  A winner will be selected from all eligible entries received by a committee selected by Sponsor on or about the 21 ^st day of each month during the Contest Period.  Decisions of Sponsor are final on all matters and decisions. Winner will be notified by email only using the information provided in the entry and will have (3) business days to respond. If there is no timely response, Sponsor may select an alternate prize winner. Limit one prize per person/household address.  All entries have an equal chance of winning. Odds of winning will depend upon the number of eligible entries received.

PRIZE:  First Prize – One (1) AmTrust Bank Kids Account funded with $150 by Sponsor.  Prize consists of only those items specifically listed as part of the prize. No substitution or transfer/assignment of prize by winners permitted . Under no circumstances will more than the advertised number of prizes be awarded.   Sponsor will work with Winner to determine most suitable AmTrust Bank branch location within which to open the AmTrust Bank Kids Account; provided, however that such Account must be opened no later than 30 days upon notification to Winner.

SPONSOR MAKES NO WARRANTY, GUARANTEE, OR REPRESENTATION OF ANY KIND CONCERNING ANY PRIZE, AND SPONSORS SPECIFICALLY DISCLAIM ANY IMPLIED WARRANTY OF MERCHANTABILITY, WARRANTY OF FITNESS, AND ANY OTHER IMPLIED WARRANTY.

All federal, state and local taxes are solely the responsibility of winner(s).  All federal, state and local laws and regulations apply.  Potential winners (or parent/legal guardian if potential winner is a minor in his/her state of residence) may be required to sign and return an Affidavit of Eligibility/Release of Liability/Prize Acceptance Form, which may require social security number, postmarked within 10 days of the date on notification materials. Noncompliance within this time period and/or return of any prize/prize notification as undeliverable may result in disqualification and an alternate potential winner may be randomly selected from all remaining eligible entries. To claim any prize, winner must provide valid government-issued photo identification satisfactory to Sponsor in its sole discretion.  Sponsor reserves the right to substitute any prize with prize of equal or greater value.
